Physical Therapist Responsibilities:

Conduct assessments of referred patients.
Deliver direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation.
Develop and implement treatment programs with tailored goals, guided by the patient’s capacity and under physician direction.
Set realistic patient goals, maintain comprehensive documentation, and educate patients and families on care plans.
Collaborate with all disciplines for optimal patient-centered treatment and care.

Physical Therapist Qualifications:

Graduation from an accredited Physical Therapy college or university.
Current and unencumbered Physical Therapy license for the State of NY.
Experience in long-term care and/or geriatric care preferred.
Proficiency in EMR documentation (specifically Wep PT).
CPR certification (Preferred).
Bilingual in English/Spanish.
