Job Opportunity: Clinical Documentation Specialist

About Us:
We are looking for a compassionate and dedicated individual to join our team as a Clinical Documentation Specialist. In this role, you will oversee the completion and approval of all clinical utilization documentation to ensure positive clinical outcomes and efficient use of resources. Responsibilities:
• Review and approve evaluation documentation, ensuring consistency with the patient’s comprehensive assessment and plan of care.
• Verify the accuracy of service utilization and alignment with assessment data to support effective outcomes.
• Process OASIS and Patient Plan of Care, identifying and addressing any deficiencies with the clinician.
• Ensure appropriate care types are selected based on the client’s medical condition and assessment data.
• Review and process ROC and Discharge Assessment Data to ensure accuracy.
• Collaborate with healthcare professionals to reassess and update the patient’s plan of care when there is a significant health status change.
• Educate clinicians on best practices in documentation and OASIS completion.
• Develop and assign training activities based on individual clinicians’ learning needs.
• Provide coaching, education, performance evaluation assessments, and support to clinicians regarding job performance.
• Collaborate with internal teams to ensure timely and appropriate processing of patient care.
• Identify and resolve issues to improve outcomes and efficiency in patient care and organizational processes.
• Participate in special projects and perform other assigned duties.

Qualifications:
• License and current registration to practice as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist in New York State.
• OASIS certification required within one year of job entry date.
• Bachelor’s or Master’s degree in a related field.
• Minimum of three years clinical experience in community health care.
• Prior clinical management, utilization management, or care management experience preferred.
• Proficiency in Microsoft Office applications.
• Demonstrated analytical skills.
