Case Manager (BSW/MSW)

A nonprofit organization that works to provide specialized care to the elderly patients enrolled in their programs is currently seeking qualified Case Managers to join their interdisciplinary team of professionals. You will work with licensed professionals in providing case management and crisis intervention services to the patients enrolled in their programs. The base pay for this position is $22 to $24 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer. Responsibilities:

Conduct comprehensive assessments and develop comprehensive service plan.
Monitor stability; identify needs and develop service plans.
Review the case management service plans and conduct reassessments.
Complete case notes and daily logs; conduct monthly support groups.
Computer literacy; able to use Microsoft, Google, and other applications.
Maintain and complete required documentation as needed.

Qualifications:

Bachelor’s/Master’s Degree in Social Work, Psychology, Human Services, or a related field.
2+ years of case management experience working with geriatric/elderly ideal.

Job Type: Full-time

Pay: $22.00 – $24.00 per hour

Monday to Friday
