An esteemed community center dedicated to providing expert social services is currently seeking qualified Case Managers for their housing programs in Brooklyn. The base pay range for this position is $20 to $21 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. Responsibilities:

Provide housing and case management services to patients enrolled in programs.
Complete comprehensive assessments, service plans, progress notes
Monitor and reassess clients’ needs for referrals and services
Work collaboratively in an interdisciplinary team to ensure comprehensive, client-centered services are delivered
Ensure all clinical and administrative documents are completed accurately
Aid with coordinating resident move-ins and move-outs
Participate in recreational events in the community to increase socialization skills
Provide support to community residents and church social programs

Qualifications:

Must have a bachelor’s degree is social work or human services related field
CASAC or CASAC-T preferred but not required
1+ years providing case mamangement/housing services ideal
Bilingual skills in English/Spanish a plus

Job Types: Full-time, Contract, Temporary

Pay: $20.00 – $21.00 per hour
